To qualify for this property you will need supported living for adults with learning disabilities. That will be provided by Creative Support, a non-profit provider of personalised services – supporting people to enjoy their lives and reach their full potential. Creative Support provide only the best for our service users. They deliver personalised care offering choice, encouraging independence, ensuring dignity and equality in a safe, happy and homely environment.  

Service provision: 7 days a week 7:00-22:00 and sleep-ins (staff are sleeping in the service every night 22:00-7:00) for emergency purposes only. The house is a short trip from the town centre (10 min bus ride), hospital and a public park; it has very good public transport connections and is close to other facilities like supermarkets and shops.  

Support offered:  

• Recovery focused personalised goal setting, such as employment and/or education 
• Budgeting skills, money management, setting up bill payments and signposting to specialist debt services 
• Benefits advice and support 
• Attending GP/hospital/other health provider appointments communities, identify suitable activities, group and peer support 
• Tenancy management and sustainability 
• Developing daily living skills such as meal planning, shopping, cooking and cleaning skills 
• Promoting healthy lifestyles, healthy eating and exercise 
• Assisting with medication administration 

They do not provide personal care but can prompt if it is highlighted in their needs assessment.  

Referral criteria: We require that people referred have a learning disabilities diagnosis, have a need to move on supported accommodation.  

Funding must be approved for individual support hours by a funding panel prior to sign up of tenancy or provision of support. Enquiries are welcome prior to panel approval.
